Applies for membership in the LADIES PENNSYLVANIA SLOVAK CATHOLIC UNION (LPSCU) and agrees with the purpose and objectives of the LPSCU, as follows: The purpose is to promote the moral, spiritual and material welfare of its members.

The objectives are: to foster the spirit and practice of true fraternalism among its members, to spread and foster the Catholic Faith, to encourage and perpetuate an appreciation of our members’ culture and heritage, to instill patriotic zeal that members remain loyal citizens of the United States, and to provide affordable insurance products, annuities and benefits to its members, through the collection and maintenance of funds.
