Youth Achievement Award Guidelines

Nominee must be an active member of the LPSCU.
Nominee must be between 13 – 18 years of age.
Prior honorees are prohibited from entering.
Nominations must be submitted to LPSCU with a postmark no later than June 1st.
Recommendation letters are encouraged.
Head and shoulder color photograph must be included with nomination form.
Fax forms or e-mails will be accepted.
All material sent becomes the property of LPSCU and will not be returned.
Decisions of the LPSCU are final.
An award will be presented to the honoree.
LPSCU Youth Achievement Award honoree may be submitted to State Fraternal Alliances having a similar award.
Appropriate letters of recommendation are encouraged to be submitted with nomination form.
